To schedule an "On-Line" Appointment please fill out and submit the following information to our hospital. Please make your "on-line" requests at least 24 hours in advance. We will check our appointment requests by 9:00 am every morning, Monday through Friday.

Carlsbad:
Mon., Tue.,Wed. & Fri.--8am - 7:45pm
Thurs & Sat.--8am - 5:45pm
Sunday--9am - 5:45pm

Vista & Escondido:
Mon. - Fri.---8am - 5:45pm
Saturday--8am - 3:45pm

If you schedule 1- 2 days in advance we will be able to accommodate your request. We will verify your appointment by either email or a telephone call.
